How to register as a Patient with the Practice
As long as you permanently reside within our practice catchment area (please refer to the map on this site) we will be delighted to welcome you as a patient.
If you have your current medical card, please bring it to the surgery with which you wish to register. If you do not have your medical card, we will give you a registration form to complete. This can be taken home for completion at your leisure if you prefer, but you should be aware that until the form has been processed by the practice you will not be registered with us. The registration forms can be completed on behalf of another person as long as they are a husband, wife or a blood relative. We regret that we are unable to accept forms completed on behalf of a partner or a common law partner.
At the time of registration, you will receive a practice leaflet which we hope you will find informative. You will also be asked to complete a new patient health questionnaire. It is very important that a questionnaire is completed for everyone age 5 and over. Please be as accurate as you can with the answers as it will help us to provide you with good medical care. Please be reassured, all information supplied is completely confidential.
In addition, as part of our registration procedure, we invite all new patients to attend a 'New Patient Health Check' with one of our practice nurses. This is an important opportunity for us to familiarise ourselves with your medical history and for you to ask any questions or discuss any concerns you may have. You will also be able to find out about us and the services we can offer you. If you would like to make an appointment, please speak to a receptionist and they will do their best to offer you an appointment that will fit into your schedule.
Once your registration process has been completed, it will probably take six to eight weeks for your full medical records to be forwarded to us from your previous practice, via the central records department.
