If you have any difficulty in accessing the digital form above – please phone Maternity Services  on 029 20 745030 during office hours.

A Midwife will contact you by telephone within 1-2 weeks of you sending the completed form, to arrange your maternity booking appointment. This will include taking a full medical history to plan your maternity care.

If you have not had a response by 14 days - please contact Maternity Services on 029 20 745030.

Pregnancy Concerns

If you have any concerns during the early stages of your pregnancy (before 17 weeks) please contact the GP Practice if you are 17 weeks+ pregnant please contact the maternity assessment unit 029 20 744658
