As part of our commitment to your ongoing health we will aim to undertake an annual review with you if you have an ongoing medical problem. This review will usually be undertaken on an annual basis in the month of your birth and you will receive a reminder  asking you to contact the surgery.

The review will normally consist of 2 stages:

1. The completion of any relevant condition(s) online review questionnaire

2. If required, we may also request you book a blood test and a face to face meeting with one of the Practice team.

It is important that you respond and complete this process so we can continue to monitor and improve your health. By completing the annual medication online review form it will allow us to review all your medication and update your repeat prescriptions for the upcoming year.
